1. What is Elevated Integrative Medicine?

Elevated Integrative Medicine is a telehealth medical practice led by Dr. Christina (Chris) Miller, M.D., specializing in integrative and functional medicine. The focus is on personalized, root-cause healing that addresses mind, body, and spirit.

2. Where is the practice located?
The office is based in Carbondale, Colorado, but all visits are conducted via telehealth.

3. Which states does Dr. Miller practice in?

Dr. Miller is licensed in 23 states:

We are able to conduct your appointment as long as you are physically located in one of our 23 licensed states at the time of your visit.

6. What are the appointment options and costs?

• New Patient Visit (60 min): $500

• Follow-Up Visit (60 min): $500

• Follow-Up Visit (30 min): $250

7. Can I book a follow up appointment if I am a previous patient with Dr. Miller?

Yes! If you are a previous patient with Dr. Miller you can book a follow up appointment with her. When booking at her new practice for the first time, be sure to select "no" when asked if you are an existing patient. This will allow us to get you set up at her new practice, CHRISTINA MILLER MD.

8. Do you accept insurance?

No, Dr. Miller does not accept insurance or Medicare. However, superbills are provided for patients under 65, which may help with reimbursement depending on your insurance plan.

9. What should I know before booking?

• You must be physically located in one of the 23 practicing states at the time of your appointment.

• All appointments must be prepaid 24 hours in advance.

• Your card will be stored securely for booking but not charged at the time of scheduling.  A separate invoice will be sent prior to your appointment date with a secure payment link. For more details please review our Office Policies and Procedures.
